The Advance Care Planning Canada initiative, led by the Canadian Hospice Palliative Care Association (CHPCA), focuses on advancing the Pan-Canadian Framework for Advance Care Planning.  

The ACP initiative works in collaboration with various sectors and professional groups to accomplish our goals.
